Explanation of Medical Forms
All current and perspective student-athletes must complete all University of Missouri Sports Medicine medical forms, complete and pass a pre-participation physical exam (from the athletic department or Student Health Center at the University of Missouri), and have Sickle Cell Testing (this may also be completed through University of Missouri Student Health Center) and results reported to the athletic department prior to participation as an eligible student-athlete or try-out.
At the bottom of the sidebar on the right-hand side of this page are electronic PDF forms that may be printed off, completed, and returned to Mizzou Sports Medicine via mail, fax, or in person. Please click on the link that refers to your situation (i.e. "New Student-Athletes" for freshmen, "Returning Student-Athletes", etc.).
